Picture a typical Monday morning at a busy practice. The phones are ringing before the doors even open. Someone’s checking in a patient, someone’s on hold with an insurance company, someone’s trying to squeeze in a walk-in. And the phone keeps ringing.

Most of those calls eventually get answered. Some don’t. And the ones that don’t rarely show up anywhere on a report — there’s no line item for “missed call,” so it’s easy to assume it’s not really a problem.

Where the cost actually shows up

A missed call from a new patient rarely becomes a callback. They call the next practice on their list instead — and you never know it happened. A missed call from an existing patient trying to reschedule often becomes a no-show instead, because rescheduling felt like too much friction. And a missed call after hours just becomes a voicemail nobody gets to until the next business day, by which point the patient has usually moved on.

None of this is anyone’s fault. Front desk staff are already stretched across check-in, billing questions, scheduling, and whatever’s happening in the waiting room. The phone is just one more thing competing for their attention, and it’s usually the thing that loses.

The pattern we keep seeing

Talk to enough practice owners and a pattern emerges: the busiest hours for patient care are also the busiest hours for the phone — which means the phone gets answered worst exactly when it matters most. Lunch rushes, Monday mornings, the last hour before close. And after hours, coverage usually drops to zero.

The instinct is often to hire more front desk staff. That helps, but it’s expensive, and it doesn’t solve the after-hours gap at all. What actually closes that gap is having something reliable answering every call — not instead of your team, but for the moments your team physically can’t get to the phone.
